 SUMMARY DESCRIPTION OF WORK

 The Heartland Housing Sanctuary program provides several different services to survivors of human trafficking, exploitation, domestic violence, sexual assault and stalking in an effort to meet them where they are at in their recovery journey. The programs services include crisis stabilization shelter, mobile advocacy, and Rapid Re-Housing services.

This position will provide support to survivors in Rapid Re-Housing services in the form of temporary financial assistance and supportive services to survivors of human trafficking, exploitation, domestic violence, sexual assault and stalking who are homeless and residing in Douglas, Sarpy, and Pottawattamie County. This is a housing first program and utilizes the progressive engagement model in providing services. The Case Manager will work to identify, screen, and serve eligible clients through the Sanctuary Program. This position will also work with a multitude of task forces and community meetings centered around housing and survivor services and Threshold, the metro’s Continuum of Care for the Homeless. To ensure that employee’s and survivors have the necessary supports and best chance at success.

Compensation: between $19.74 and $23.00 per hour (wage is determined by total years of relevant experience)
Work Schedule: 37.50 hours per week

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree in Social Work or Human Service field, and two years direct care experience in Human Services preferred; or equivalent

Valid Driver’s License and acceptable driving record

Essential Duties and Responsibilities

  • *Responsible for verifying and documenting the eligibility of applicants and maintaining comprehensive, accurate service records for all approved or denied clients.
  • *Conducts interviews and assessment to accept, deny, and refer applicants.
  • *Assists the clients in identification of strengths, needs and goals. Develops an individualized housing stability plan in cooperation with the client.  Monitor and evaluate program participant progress.
  • *Provides problem solving and crisis intervention services to empower clients to reach self-sufficiency.
  • *Accurately and thoroughly complete all required paperwork and data entry per agency policies and funding guidelines and within prescribed timelines.
  • *Demonstrates knowledge of resources that are available to survivors and is interested in engaging in training opportunities in order to increase knowledge to meet client needs.
  • *Demonstrates ability to help clients effectively by referring to and utilizing agency and community resources to obtain self-sufficiency.
  • *Work is completed in client homes, homeless shelters, office space, and other non-traditional settings. Ensure safety and security measures for in-home/outreach services are followed.
  • *In town travel is required, includes use of personal vehicle.
  • *Participate in weekly team meetings with HFS program staff.
  • *Creates, maintains and shares as appropriate a dynamic self-care plan.
  • * Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • *Can work both independently, with little supervision, and as a part of a team.
  • *Able to establish regularity and dependability in scheduling all appointment and accuracy in reports and statistics.
  • Abides by all specific program and Agency policies, procedures, and requirements.
  • Completes all required monthly, quarterly, and annual reports for quality improvement and grant purposes.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
  • Develops positive working relationships with clients, referral sources, service agencies, and others encountered during work.
