The Community Health Navigator is someone who is passionate about community, understands the importance of addressing social drivers of health to help the community to thrive where they live. Our non-clinical space, the UNMC/NM Community Wellness Collaborative will serve as support and as a resource to the Highlander Community and North Omaha in order to expand health access and build career pathways.

Shift Details:

M-F 8am - 4:30pm

*May be needed to work occasional evenings and weekends for community outreach.

Job Posting Details

Serve as a resource at the UNMC/Nebraska Medicine Community Wellness Collaborative and on campus as directed in obtaining/sharing available resources for the community to address social drivers of health to improve their health and wellness.

Required Qualifications:

  • Bachelor's degree in human services or related field or equivalent combination of education/experience required.
  • Minimum one year experience working on social drivers/determinants of health and available resources within Douglas County required.
  • Completion of Community Health Education didactic training or experience with SDOH required.

Preferred Qualifications:

  • Master's degree in human services or related field preferred.
  • Broad knowledge of available local, state and federal programs for assistance in meeting basic needs (food, housing, access to health care, transportation, Medicaid/Medicare, drug assistance programs) preferred.
  • Knowledge of health care and mental health/substance use programs in the area preferred.
